1.AMM-661-Implemented the addition of "Revisit & Refer" section for the visit category "General OPD (QC)"  in doctor screen.

2.AMM-754-Implemented the addition of "Revisit & Refer" section for the visit category "General OPD (QC)"  in doctor screen.
