Nurse Section

Visit Details 

  • The nurse conducting the visit. It ensures accountability and tracking of patient care by the specific healthcare worker.
  • Chief Complaints:
    • Chief Complaints: A brief description of the primary health concerns reported by the patient during the visit.
    • Duration: Enter the duration for which the patient has been experiencing the chief complaints .
    • Unit of Duration: Select the unit of time for the duration
    • Description: A detailed description of the complaint to provide additional context.

 Adherence

  • Drugs: Track whether the patient is adhering to prescribed medications, including any issues or deviations from the plan.
  • Referral: Record if the patient is referred to another healthcare facility or specialist. 

Investigations

  • Laboratory: Document any laboratory tests ordered during the visit, including tests related to pregnancy, infections, or general health checks.

Upload Files

  • Select File: Allows the user to select files (such as lab reports, prescriptions, or any other relevant documents) from the device.

2.ANC 

A.ANC Details

This section captures critical maternal health data for pregnant women. It is structured as follows:

  1. Primi Gravida:
    Indicates if the woman is pregnant for the first time.
  2. Trimester Number:
    Displays the current trimester (1st, 2nd, or 3rd) of the pregnancy.

B.Obstetric Formula

The obstetric formula helps track a woman's pregnancy history using the following parameters:

  1. Gravida (G = P + A + 1):
    Gravida refers to the number of pregnancies a woman has had, including the current one. It is calculated as the sum of:
    • P (Para): Number of live births
    • A (Abortions): Number of abortions or miscarriages
    • Plus 1 for the current pregnancy.
  2. Para (P):
    Represents the number of pregnancies that have progressed to a viable gestational age (resulting in live births or stillbirths).
  3. Abortions (A):
    The number of pregnancies that ended in abortion or miscarriage.
  4. Stillbirth (S):
    Records the number of pregnancies that resulted in a stillbirth.
  5. Living Children (L):
    Displays the number of living children the woman has.
  6. Blood Group Type:
    The blood group of the mother (e.g., A, B, AB, O) and Rh factor (positive or negative).

C.ANC Immunization

This section tracks the immunization status of pregnant women, particularly for tetanus and diphtheria (Td) vaccinations.

  1. Td-1 Status:
    Indicates whether the woman has received the first dose of the tetanus and diphtheria (Td) vaccine.
    • Date Received: The date on which the first Td dose was administered.
    • Facility Name: The name of the healthcare facility where the vaccination was given.
  2. Td-Booster Status:
    Shows if the woman has received a Td booster dose, typically administered if required by vaccination guidelines.
    • Date Received: The date the booster was given.
    • Facility Name: The name of the facility where the booster was administered.
  3. Td-2 Status:
    Indicates whether the second Td dose has been received.
    • Date Received: The date the second Td dose was administered.
    • Facility Name: The name of the healthcare facility where this vaccination was provided.
